Hospice Eligibility
Hospice is a type of specialized care for individuals who have a terminal illness or a life-limiting condition and have a prognosis of six months or less to live. This can include individuals with cancer, heart disease, dementia, ALS, HIV/AIDS, or other terminal illnesses.

To be eligible for hospice, a person must meet certain criteria, including:
- A physician must certify that the person has a terminal illness and a prognosis of six months or less.
- The person or their legal representative must sign a consent form stating that they understand the focus of hospice care is on comfort and symptom management rather than curative treatment.
- The person must agree to forgo curative treatment for their terminal illness while receiving hospice care.
- The person must be willing to receive care in the location where they reside, which can include their home, a hospice facility, or a nursing home.
It is important to note that hospice care is a personal choice and individuals may choose to discontinue hospice care at any time if they feel it is no longer appropriate for them. Hospice providers work with individuals and their families to develop a care plan that meets their unique needs and preferences.
